Welcome Ariadne Labs to the Honoring Choices MA Partner Network!
We are delighted to announce that Ariadne Labs has become a Honoring Choices MA Alliance Partner. We are working collaboratively with the Ariadne Lab’s Serious Illness Care Program to introduce their proven Serious Illness Conversation Guides to the health care providers and community professionals in the Honoring Choices Partner Network.
Ariadne Labs is a joint center for health systems innovation at Brigham and Women’s Hospital and Harvard T.H. Chan School of Public Health. They develop simple, scalable solutions that dramatically improve the delivery of health care at critical moments to save lives and reduce suffering. Their vision is for health systems to deliver the best possible care for every patient, everywhere, every time. Ariadne Lab’s programs and projects are at varying stages of development, from prototyping to wide-scale dissemination around the world.
Together with Ariadne Labs, Honoring Choices Massachusetts (HCM) is offering a new informative webinar, aligning the HCM Getting Started Tool Kit with Ariadne Labs Serious Illness Conversation Guide, to provide the HCM Partners with a seamless framework for engaging adults in proactive planning conversations at any phase of health. The webinar offers front-line, multi-disciplinary care providers a best communication approach to start simple “Who’s Your Agent’? conversations using the Getting Started Tool Kit, and confidently flow into more complex “What matters most?” conversations, using the Serious Illness Conversation Guide. Aligning our programs and tools helps consumers and health care providers engage in early and effective conversations, and complete MA care planning documents to help ensure patient goals, values, and choices are understood and honored.
Mass Home Care, Home Care Alliance of Massachusetts, and other HCM Partners have already provided this live, interactive webinar to their staff and care teams. Nurses, Social Workers, Care Managers and other care providers report the webinar has greatly added to their confidence, knowledge and skill level, and is particularly timely and useful in light of Covid-19. We will be offering a more comprehensive Serious Illness Conversation Guide training to the Honoring Choices Partner Network shortly.
