Here's a short video that shows you how to complete your own Honoring Choices Health Care Proxy. Download the MA Health Care Proxy (English) and follow along with the video.

IMPORTANT: You will need two people to act as your witnesses to complete your Health Care Proxy. Your witnesses can be any adult, 18 years old and older, but not the person you choose as your Health Care Agent and Alternative Agent. Your witnesses are present to watch you sign and date your Health Care Proxy, and each witness will sign and date the document right after you.
