Tribute Home Care Reaches Out to Community Members

“We had a terrific exchange of information and ideas,” says Amy Winning, Director of Community Relations,  “and the group left feeling more empowered about their right to communicate what they want for their health care.”  As an Honoring Choices Community Partner,  Ms. Winning of Tribute Home Care in Charlestown,  provided a “Who’s Your Agent?” presentation to the interested residents of Clarendon Hill Towers in Somerville.  The presentation provides information and tools to make a personal health care plan. The discussion was lively and focused on “each person’s choice to direct his or her own health care”, said Amy, “and the importance of choosing a trusted person in a Health Care Proxy to speak for you if you are unable to speak for yourself.” The presentation covers the 5 Massachusetts planning documents, a Health Care Proxy, Personal Directive (Living Will), Durable Power of Attorney, MOLST (Medical Orders for Life-Sustaining Treatment) and CC/DNR (Comfort Care/ Do not Resuscitate Order).  Amy offered specific tools and resources to enable attendees to take the next steps in creating their own personal plan.  Thank you Amy and Tribute Care for reaching out to your fellow community members to help inform and empower individuals to make a health care plan and connect to person-centered care all through their lives.
