Honoring Choices and Tufts Health Plan Foundation on a Shared Mission
We are delighted to take the next step in our collaborative mission with a generous grant from the Tufts Health Plan Foundation and launch the Honoring Choices Massachusetts Who’s Your Agent?® Ambassador Program.
The Ambassador Program empowers community volunteers to engage adults in peer-to-peer health care planning discussions within their own language, culture, religion and ability. Ambassadors receive training in the Honoring Choices Who’s Your Agent?® Program to bring health care planning discussions to adults and families where adults live, work and gather.
Our goal is to promote thoughtful discussions and access to lifelong wellness that transcends the barriers of age, language, cultural, race, religion, income, and one’s ability to make and communicate decisions.
Working with our Community Partners, Ambassadors serve as a bridge to start comfortable person-to-person conversations, and provide vital consumer feedback to our Partners to help create better access to quality person-directed care. The Honoring Choices Community Partners will initiate Ambassador Programs at senior centers, faith communities, within cultural communities, patient-family advisory councils, dementia friendly programs and anywhere consumers gather.
We are thrilled to be working with Tufts Health Plan Foundation on our shared mission of addressing health inequities to help ensure every older adult has easy access to health care planning information to make a personal plan and get quality care to live a healthy, fulfilling life.
