POINT32 HEALTH’s Equity-focused Advance Care Planning Initiative
Guiding and empowering healthier lives for everyone- POINT32 Health’s mission- can often start with a simple planning conversation. Working with Lauren Hartwell, MPH Senior Program Manager Population Health Capabilities and her colleagues this past fall, we tailored the Honoring Choices MA (HCM) Who’s Your Agent? Program for Care Providers to support an inclusive, system-wide Point 32Health Advance Care Planning Initiative. We trained 175 staff as Health Care Ambassadors to start simple conversations using the HCM Getting Started tool Kit, and 135 staff to engage adults in serious illness conversations using the HCM Next Steps Tool Kit and introducing Ariadne Labs’s Serious Illness Conversation Guides. We are also working with Martha MacDougall, RN, BSN, CCM, Senior Manager, Clinical Provider Performance, to inform care provider teams about the initiative and offer the same tools and training to their individual organizations and their staff. And that’s just our first steps! We look forward to building on the progress to date so all POINT32Heath members have equal access to health care planning tools and can get connected to timely appropriate care.
