"MOLST: Meeting the Challenge" Workshops for Care Providers

We are co-hosting a series of workshops for care providers. Each workshop brings together a dynamic panel and multi-disciplined care professionals for a powerful discussion about best practices, gaps in care and aligning our protocols and procedures. Participants share their expertise and generate a priority list of "next steps", which will be captured and summarized in a final "Outcomes Report". See Workshops information below.

Who Can Attend: All care professionals are welcome. Doctors, nurses, social workers, EMS, care managers, attorneys, clergy — all care professionals from across care and community settings. We are delighted to offer CE Credits to Emergency Medical Services.

"Let's Talk About MOLST" Workshops For Consumers

Tell us about your experience or your loved one's experience with MOLST and living with serious illness.  Are you getting the care you want — no more and no less?

We invite adults, family members, Health Care Agents, Guardians and Caregivers to offer feedback and suggestions to ensure all care providers can know and honor your choices using a MOLST form. We'll share the latest information & tools, and create a "listening session" to carefully understand the consumer and family experience. Together. we'll generate a list of 'next steps' to help improve every phase of the serious illness journey and end of life care. Starting in the Fall. Details to follow.
