The Who’s Your Agent?® Program is the first public health education and engagement program to empower competent adults, 18 years old and older, to make a personal health care plan to get the best possible care at every phase of health.

Your Health Care Plan  is your road map to care that protest your right to get the care you want, helps your family know how to care for you, and tells your doctors how to match care to your goals, values and choices.

We’ve taken all the information and tools in the  3 Step Health Care Planning Guide and made two easy-to use tool kits to make you own health care plan. The Getting Started Tool Kit helps you or your loved ones to start planning discussions with their Agent, family and care providers. The Next Steps Tool Kit builds on your discussions and adds planning documents over your life time.
