LIVE WEBINAR: Palliative Care: Aligning the Team Around the Patient

On Tuesday, June 19, 2018, Honoring Choices is co-sponsoring a unique workshop for all clinicians and care providers along with Honoring Choices Partners the Massachusetts Medical Society, American Cancer Society, Hospice & Palliative Care Federation of MA and Lahey Health. Many physicians and clinical care teams have not received formal training in palliative medicine yet are being asked to initiate palliative care discussions with patients diagnosed with a serious illness. The webinar will offer primary and specialty clinicians a set of key questions to open a palliative care discussion with patients and families at any phase of serious illness, from first diagnosis through to end of life care, and suggest when and how to refer to specialty palliative care where appropriate. The webinar provides a useful framework of the 5 phases of serious illness and how palliative care can be an effective layer of support at each phase and as a patient’s illness advances. Participants will takeaway a common set of terms and definitions their care team can use with patients,  and receive a set of planning tools and discussion guides. The webinar will also introduce the new Palliative Care Directory for Massachusetts, recently launched by Hospice & Palliative Care Federation of Massachusetts.
