Your donation helps us provide every adult in the Commonwealth with no cost downloadable MA planning documents and tools to make a personal plan.

Your gift also helps us support thousands of health care providers, the Honoring Choices Partners, as they work to standardize care planning and connect adults to good care in their community.

Any amount is most gratefully received. You can choose to make a donation either-

Offline donation via mailing a paper check to Honoring Choices MA.
