Commonwealth Care Alliance Partners with Honoring Choices

We are excited to welcome Commonwealth Care Alliance® (CCA) as an Honoring Choices Massachusetts Partner. CCA’s mission is to improve the health and well-being of people with significant needs by innovating, coordinating, and providing the highest-quality, individualized care. CCA’s unique model of uncommon care® delivers better health outcomes through seamless integration, a community focus, and an emphasis on the behavioral and social determinants of health that account for so many critical gaps in care. This unique model successfully engages hard to reach individuals, and helps those with complex needs live safely and independently in their homes and communities for as long as possible.

As an Honoring Choices Partner, we will support CCA’s dynamic approach to advance care planning for their Senior Care Options (SCO) members, adults 65 and older. Working collaboratively with Wayne S. Saltsman, MD, PhD, Senior Medical Director, SCO Program, Honoring Choices will help tailor an initiative to reach every SCO member, especially the highest risk individuals, to engage in routine conversations to make their own personal plan starting with the appointment of a Health Care Agent in a Health Care Proxy. We will work closely with Dr. Saltsman and the CCA Advance Care Planning Task Force to design a comprehensive program that supports the CCA care teams as they engage in effective goals of care conversations to provide the best possible care over an individual’s lifetime. In addition, Honoring Choices materials and tools will be available to all CCA members and their care teams.
