Honoring Choices Massachusetts (HCM), Ariadne Labs and The Conversation Project are working together to help ensure adults and their health care teams can confidently engage in effective planning conversations from simple to serious illness. Our programs work together seamlessly to provide a continuum of planning tools and conversation guides that help adults make a personal care plan, talk with their family and other important people about their care, and work with their health care teams to get quality care that aligns with their goals, values and care choices.
