Session Three of Envisioning Community Based Palliative Care in MA series, hosted by The Palliative Care Advisory Group, Honoring Choices Massachusetts.
Join us for a unique presentation followed by a moderated panel discussion as we explore the Eight Key Elements of a Community Based Palliative Care Approach in MA, a new collective framework generated by MA experts assisting individuals with serious illness and national palliative care leaders.
The collective framework supports a community based palliative care (CBPC) approach across all settings to help patients when and where they need it, and shepherd patients & families through the care continuum. The framework’s shared goals offers a flexible, high quality scope of services to: 1) meet a wide diversity of patient needs, 2) prepare an interprofessional workforce, and 3) advance organizational missions.
A panel of diverse experts will have a candid discussion about-
- Similarities, differences, and gaps in a CBPC approach
- In-house challenges & structural barriers
- “What we have and what we need” to support a CBPC approach within primary, specialty, community, psychosocial, home care, and hospice settings.
Takeaways. Participants receive on-line educational resources to equally train staff and educate consumers and more.
Who can attend. All interprofessional care team members who provide community-based care including primary or specialty palliative care, psychosocial support, home care, and hospice.
