Honoring Choices MA Partners & Friends are celebrating the 13th Annual Health Care Decisions Spring Celebration, in honor of April 16, National Healthcare Decisions Day. All adults, 18 and older, can start to make their own personal care plan by downloading a free MA Health Care Proxy, a handy wallet card and Personal Directive (Living Will). Community groups and care professional organizations can co-host a fun and informative webinar with Honoring Choices. Just choose a date; we provide a virtual presentation with no cost tool kits.
