12-Stop Tour: Who’s Your Agent? Workshop for Care Providers

Honoring Choices Partners are taking the lead to arm their care providers and staff with a common, unified health care planning language & set of Massachusetts tools, to actively engage adults & families in health care planning discussions.

The Who’s Your Agent? Health Care Planning Workshop for Care Providers fall tour kicks off  September 15  in Cape Cod, and runs through the end of the year in every region across the Commonwealth. The Workshops help care providers and staff to enhance their knowledge and sharpen their communication skills to effectively engage adults in planning discussions and connect adults to person-centered care.

The Alliance Partners Train-the-Trainer Workshop, invites care professionals from Alliance Partner  member organizations to become “Champions”. Champions gain knowledge and skill to present the “Who’s Your Agent? Workshop” to their fellow members, care providers, and consumers statewide.

The Community Partners Workshops supply care providers and their staff  with the NEW  Who’s Your Agent?  “Getting Started” and “Next Steps” Tool Kits to actively engage adults & families in discussions. The Community Partners come together in regional workshops to build a strong continuum of person-centered care for adults in their community.
